Bisphosphonates and denosumab in dentistry

Dental review for patients receiving bisphosphonates or denosumab should identify the antiresorptive treatment and its indication. Novik supports case organisation and evidence review without replacing individual assessment.

Why are these treatments reviewed?

Antiresorptives are associated with medication-related osteonecrosis of the jaw (MRONJ). The ADA describes this as rare but serious in osteoporosis treatment. Oncology treatment requires specific assessment and should not automatically be treated as equivalent to osteoporosis therapy.

What information does the dentist need?

Record the active medicine, indication, administration route, treatment duration and dental history. Specify whether an extraction, implant or another procedure is planned. Include other relevant treatments and medical conditions.

Does a medication break remove the risk?

This cannot be assumed. The ADA notes that discontinuing bisphosphonates may not eliminate the risk and may adversely affect treatment of bone disease. Responsible clinicians make these decisions. AI alone cannot establish surgical suitability or determine medication changes.
